Apart from the inconsistent lines and missed apexes that Marshy pointed out, I think you consistently went too wide into 2, come out of 2 a bit wide which puts you into 3 too tight and come out of 4 too wide which messes up your entry to turn 5. As far as the highside goes, it sounds like you got on the throttle fairly aggressively while you still had a lot of weight on the front tyre (due to turning harder from the narrower angle of entry) and it spun up and slipped out due to the lack of weight on the rear tyre.
